The PPD will let you choose any paper size. Here I'm saving 13.5x13 centered on a 30x15 page

Note that the PPDs folder is case sensitive—lowercase s

That is best done by exporting and if an inhouse booklet printing is needed, use Acrobat's booklet feature instead.
Imposition is the job of the printer, not a designer.

rob day Apr 6, 2016 1:53 PM (in response to Willi Adelberger)My point is we don't know all of the variables that any poster faces—they may need simple imposition for a one color job where transparency and CM aren't an issue and can't control what happens downstream. It is fair enough (and important) to clarify that imposition is usually the printer's responsibility, but it is a generalization and there could be an exceptions.
With the correct PPD I haven't had any problems with print booklet on OSX—is there a reproducible bug?

BobLevine Jul 10, 2018 11:38 AM (in response to marklarson)The whole thing is nothing but one huge kludgy workaround. It is not a feature and is based workflows that have been deprecated for many, many years. Export a proper PDF and then print that from Acrobat using the booklet feature there.
If this is being demanded or requested by a printer, find a new one. This one doesn’t want to do their job.

I can print booklet from Acrobat, but I need it as a pdf and I can't print booklet to pdf from Acrobat, which blows my mind a bit...

The Adobe PDF 9.0 PPD is still working for me with CC2018. If you have recently upgraded you have to install the PPD and its folder in your InDesign's Presets folder. It's not a workaround, you are trying to print/save a Postscript file, so you need the proper PPD.

I can't charge for this time so I've stopped working on it and just sent off the package and a print-ready .pdf. I'd rather know how to do it, but I don't have a whole day to try and figure it out. Any insight you could give me would be greatly appreciated. Thank you for the quick response and friendly demeanor. That is also greatly appreciated.

rob day Jul 10, 2018 12:25 PM (in response to marklarson)Print Booklet is not a very robust imposition feature and there can be problems with the postscript workflow, because it lacks complete color management and transparency gets flattened. It really is intended for making in-house dummies from composite printers.
But if the PPD is properly installed you should be able to select it in the Print Settings... dialog:
